Seminars
Visionary Ophthalmology offers Continuing Medical Education credit seminars for optometrists. If you are interested in attending our seminars, please let us know via the form at the bottom of this page so we can add you to our mailing list. In addition, we offer seminars for clubs, groups, and schools about eye health.
Complimentary Screening Visits
Visionary Ophthalmology provides complimentary screening visits in order to determine if you are a candidate for LASIK or other Refractive procedures. The screening visits are conducted by our technical staff Monday through Friday between 8:30 a.m. and 4:00 p.m. Should the screening uncover a problem, we shall be pleased to schedule a follow up examination with one of our doctors to determine a treatment plan.
Insurance Plans
Visionary Ophthalmology participates with most major insurance plans. Please bring your photo identification, current insurance card, and referral (if required by your plan) to your appointment so we may assist you in filing for your insurance benefits.
Fees for Miscellaneous Services
- MVA Form – $15 – Filled out by Technician-Optician
- Disability Letter – $30 – Filled out by Technician
- Workers Comp Letter – $50 Short, $70 Long – Filled out by Technician
- Medical Necessity Letter – $35 – Filled out by Technician
- Letter to be Dictated by Doctor – $60 Short, $85 Long, $350 Immigration, $375 Lawyer – Filled out by Doctor
- Letter to be Dictated by Technician – $45 Short, $65 Long, $225 Immigration, $225 Lawyer – Filled out by Technician
- Record Release – $15, Plus+ $.50 per page and Shipping & Handling – Released by Technician
- Phone Conversation with Doctor – $800 per hour, prorated per minute with Lawyer – Doctor
- Other – To be determined by Doctor/Manager
